Is Iqama considered necessary only for Fard rakats or for non-obligatory rakats also?

If Iqama is only before fard rakats, then what if sunnah rakats are to be offered before fard as in Dohr salat?

Also, is Iqama in the Quran?

If sunnah rakahs are to be offered before fard, then begin sunnah without Iqama and then recite Iqama prior to starting the fard rakahs.  From what I know, this is the concept followed by traditionalists. 

Iqama, the second call for prayers, is not in the Quran.  I don't know of any Verse that specifies Iqama.   However, it's a good practice and in congregations it's also helpful so everyone can know when the prayer commences.   But for people residing alone in their homes, Iqama would not have any practical purpose. They only need to hear the Adhan to know the starting time for every prayer.   In Western countries, the prayer calendar is usually substituted for Adhan.

Adhan, the first call for prayers, is in the Quran with reference to Jum'ah prayers.   

"O you who believe!  When the call is heard for the prayer of the day of congregation, haste unto remembrance of Allah and leave your trading.  That is better for you if you did but know."  62:9  Al-Jum'uah. 

From Verse 62:9 one can assume that Adhan before prayer time is obligatory before all five prayers.  Adhan is only made from the Masjids.
